The ‘Critical 5’ priority areas were launched earlier this year for Surrey Heartlands. Our Guildford and Waverley place objectives relate to:

  • Keeping people well – community engagement and people health management approaches, developing thriving communities, preventing patient deterioration, developing non-acute urgent care models and working with communities on self-care
  • Safe and effective discharge – unlocking Surrey (patient discharge process) - programme delivery
  • High-Risk-Care-Management – development of end-of-life care model, optimising planned pathways - end-to-end, diagnostics and community models
  • Effective Hospital Management – deliver strategy priorities, including integration of pathways and frailty
  • Managing efficiencies across the Surrey Heartlands system
